Important Announcement: After 6 wonderful years in her Pawtucket Rhode Island office, Dr. Behtash closed her private practice in July of 2019.

In September 2019, Dr. Behtash will begin seeing Medicare Patients at Oak Street Health. She will begin working in Warwick and transfer to Providence when the office is available. If you are a medicare patient and would like to follow Dr. Behtash to Oak Street Health, please call Oak Street Health at (844) 253-4657 .

If you are not a medicare patient, you will unfortunately need to find a new primary care provider. Dr. Behtash is currently referring patients to the following providers who are accepting new patients. These practices use the same electronic health record system, so it should be very easy to get established into the new practice.

If you transfer to another primary care provider, who is not on the Care New England Epic Medical Record System, you can request a copy of your medical record by faxing a medical records release authorization form to (401)489-7887. Your new provider’s office should provide the form.
